Organization Funds Request

Please follow these steps to request funds for an individual

  • Fill out this form as much as possible.
  • Click the SUBMIT button on the bottom of this page.
  • An email will be sent to you and to the Clear Lake Lions recapping your request.
  • All 501(c)(3) & 501(c)(4) organizations must submit an IRS tax status determination letter, or have one on file with the Clear Lake Lions.
